Oyate Health Center (OHC) is a tribally owned and operated primary care clinic located in Rapid City, South Dakota. We are proudly managed by Great Plains Tribal Health on behalf of the Oglala Sioux Tribe, Cheyenne River Sioux Tribe, and Rosebud Sioux Tribe—because our relatives deserve healthcare that reflects our values, our strengths, and our sovereignty.
OHC exists because tribal nations and community members pushed for something better: high-quality healthcare that is culturally respectful and accountable to the people it serves. Who We Serve
Oyate Health Center provides health and wellness services to American Indians and Alaska Natives. Anyone who is eligible for services through the Indian Health Service (IHS) may receive care at OHC.

Medical Home
In a medical home, a primary care provider serves as the main point of contact for a relative’s healthcare needs. The care team works together to provide accessible, continuous, and personalized care. We focus on building a relationship with you.
Our goal is to keep you healthy and help you manage any health problems you may have.
This model focuses on building strong relationships between relatives and their healthcare providers. The model promotes preventive care and manages chronic conditions. It addresses the physical, mental, and emotional needs of relatives.
