Our primary care is composed of medical teams to provide high-quality care for all ages for acute, chronic, episodic and preventive needs. Our team will coordinate with specialists through and internal and external referral process to ensure all needs are addressed. In most cases, the conditions we treat are not severe or life-threatening. Our teams consist of Primary care providers, case and care manager nurses, a scheduler, women’s health provider, pediatric providers, pharmacist, behavioral health consultant, dietician and diabetic educator. This comprehensive approach is here to enhance your health.

Primary Care Teams
Our Primary Care teams are organized by location and include providers, nurses, and case managers who work together to support coordinated patient care.
Each team is named after a traditional Lakota medicine that is still used today honoring long standing knowledge practices that support healing balance and wellness within Lakota communities.
- Primary Care North teams are located on the north side of the clinic and focus on ongoing primary care and long term health management.
- Primary Care South teams are located on the south side of the clinic and provide both primary care and same day care services.
Patients are assigned to a care team based on their needs to ensure comprehensive and culturally responsive care.
