We provide diabetes related education services and activities. Our team includes a certified diabetes care and education specialist, a registered dietitian, a JVN acquisition specialist, and lifestyle coaches. We draw on shared knowledge from OHC medical staff, including dentists, nurses, podiatrists, providers, and optometrists.
Population Served:
Any relative 18 & over that is a beneficiary of the Oyate Health Center
Services offered:
- Nutrition education, diabetes education, JVN diabetic eye screenings.
- Referrals to primary care providers, walk in clinics, urgent care, and self referrals.
- Bus passes for relatives with diabetes or prediabetes to attend appointments.
- Lifestyle coaching and exercise goal setting.
- In house challenges and monthly newsletters.
- Workouts of the day and swim center passes.
- Incentives for relatives with prediabetes or diabetes.
Criteria For Program:
American Indians/Alaskan Natives ages 18 and over living in the Rapid City area who have a diagnosis of Diabetes or Pre-Diabetes.
