Oyate Model of Care
Integrated Care
We have embraced an integrated care model known as the Oyate Model of Care. This model is centered around our Relatives and is designed to be comprehensive, accessible, and continuous. Our goal is to build strong, supportive relationships between the care team and our Relatives and their families. In this patient-centered approach, we encourage and empower our Relatives to actively participate in decision-making alongside their care team.
Primary Care
Primary Care services are available from 8:00 am to 4:30 pm, with some appointments starting as early as 7:30 am and extending as late as 5:30 pm, Monday through Friday (excluding recognized holidays). We encourage all Relatives to establish a care relationship with a primary care provider. Our providers deliver high-quality medical care for acute, chronic, and preventive needs, and can coordinate care with specialists through a referral process. In most cases, the conditions we treat are not severe or life-threatening, and the primary care provider is typically the first point of contact for our Relatives.
Same Day Care
For immediate concerns, our Same Day Care is available from 7:00 am to 7:00 pm, seven days a week. This service is designed for non-life-threatening conditions and is provided on a walk-in or call-in basis. Please note that OHC does not have an emergency room. When our center is closed, a Nurse Call Line is available from 7:00 pm to 7:00 am every day. By calling our main line (605-355-2500) after hours, Relatives will be connected with a Registered Nurse who can offer advice on health questions or concerns or direct them to an emergency room if necessary.

Who Qualifies?
The Oyate Health Center serves the Native American community of Rapid City and eligibility requirements for care are the same as the Indian Health Service.
The Indian Health Service is the health care system for federally recognized American Indian and Alaska Natives in the United States .
The policies, standards, and procedures that determine if you can get care at an Indian Health program are in the Indian Health Manual Part 2, Chapter 1. Specific questions about getting health care can be answered by our staff in patient registration at the Oyate Health Center.
You may also be able to get care at tribal health programs based on the Tribe’s agreement with IHS. You will have to contact these tribal health programs individually regarding health care being provided to a non-affiliated tribal member.

Parking and Entrance
Enter through the East Entrance, facing Canyon Lake Dr. The West entrance of the building, off Soo San Dr., is a locked OHC staff entrance. The East parking lot is convenient for Relatives to access the building.
