Creating a Chart at Oyate Health Center
Registering as a patient at Oyate Health Center is simple. Visit the Registration Desk located in the main lobby, where new Relatives will receive a registration packet to begin the process.
Required Documentation for a New Chart & Oyate Care Eligibility
To establish a chart and determine eligibility, new Relatives must provide the following:
#1. Tribal Enrollment or Descendancy Documentation
- Relatives must bring one of the following:
- Certificate of Indian Blood (CIB), or
- Tribal ID Card
If neither document is available, a Release of Information (ROI) may be signed, allowing Oyate Health Center to request Tribal Enrollment documentation directly from the Tribe.
If a Relative is not enrolled but is using descendancy from a tribally enrolled family member, they must provide:
- The birth certificate of the enrolled tribal member, and
- That member’s tribal enrollment document.
#2. Birth Certificate
- New Relatives must provide a birth certificate during registration. If a birth certificate is unavailable, a South Dakota Driver’s License with a Gold Star may be used instead. The license must be valid and unexpired.
All required documents must be submitted within 30 days of the initial registration date.

Pennington County Residents
If the Relative resides in Pennington County, proof of residency (POR) must be provided. Acceptable documents include the following:
- Current Mortgage or lease agreement
- Current Utility Bill (Water, Electric, Gas, Heat)
- Current residential Land Line phone, Cable or Internet Bill with service address
- Current unexpired Driver’s License or State ID (must have gold star)
- Unexpired Tribal ID with Pennington County SD address.
- Prior year tax documents – accepted by IRS Voter Registration within the last year
- Vehicle Insurance Records or Vehicle Registration Records current in Pennington County, SD
- Current Federal/State/County mail (e.g. IRS, SD DSS, SSA)
- Current Pay Stub showing proof of employment and employers contact info.
